Person-centered planning is about the individual’s future and reaching their goals. The process should be driven by the individual and reflect what is important to and for that person. Person-centered planning can include other people, such as family or friends, only if the individual chooses to include them in the process. The individual choices decided through person-centered planning about what the participant wants and needs and what services and supports will help them reach their goals is used to inform their individual program plan (IPP) with their regional center.

Holistic Consultants Staff are trained Independent Facilitators and are able to assist with writing an individual’s person-centered plan and budget. We will walk you through the process from start to finish.

Holistic Consultants will assist you with the development of your person-centered plan and can stay on if you request for on-going Monthly support. We can also assist with coordination of Services to meet the tailored needs of the individual.

CFS is a new service option specifically designed for adults who are 18 years and older who: Choose to live in their family home Are served by a regional center CFS provides assistance with the coordination of services and supports that allow adults to continue living in their family home such as: Developing skills in the home Coordinating & getting services delivered, including help with “generic services” Helping people access their community

Holistic Consultants Staff will reach out to the individual and family with in 72 hours of the initial referral

CFS is for adults (18 years and older) who live with their family and are receiving services from a Regional Center.
